This Notice describes how Albuquerque Guidance Center may use and disclose your protected health information (PHI) and explains your rights regarding that information. We are required by law to maintain the privacy of your PHI and to provide you with this notice.

Protected health information may be used or disclosed for treatment, payment, healthcare operations, and as otherwise required or permitted by law. Certain uses and disclosures, including most marketing uses or the sale of PHI, require your written authorization.

- You may request an amendment to your records.
- You may request restrictions on certain disclosures.
- You may request confidential communications.
- You may file a complaint if you believe your privacy rights have been violated.
Website Privacy Practices
Any public contact form should collect only non-PHI information, such as your name, email address, and a general message. Sensitive clinical or medical information should be shared only through a secure HIPAA-compliant intake platform.
All website pages should be served over HTTPS. Any third-party tools that may encounter protected health information must be supported by a signed Business Associate Agreement where required.
